3. Attach the Photographs in the Relevant Spaces in the Table.

You can copy the Photographs from MS PAINT or PHOTOSHOP, and Paste into the Given Area

OR

in MS WORD 2000 or higher, use Insert >> Picture to insert the Phtotographs.

4. Save the File for your future use.
